Description: Based upon review of five children's records and provider interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to each child's attendance or within 30 days after the first day of attendance. Evidence: 1. According to the records provided, children 1 and 2 began attending on 8/22/2016. There was no documentation of physical examinations for children 1 and 2. 2. According to the record provided, child 3 began attending 3/1/217. There was no documentation of a physical examination. 3. According to the record provided, child 5 began attending 1/16/2017. There was no documentation of a physical examination. 4. The provider was unable to provide documentation of physical examinations for the above children.

Plan of Correction: The provider responded with the following: All parents will be required to obtain documentation of a physical examination within 30 days of the child's attendance.
