An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 12:05 PM and departed at 2:25 PM. Lunch service and nap time were observed. A sample of 5 caregiver/household records and 5 children's records were reviewed. it was reported that no medication is administered to children n care and no children's injuries were reported. The pet record and chimney inspection were reviewed. Emergency preparedness procedures and drills were reviewed. An exit meeting was conducted with the provider prior to closure of the inspection. Description: Based upon review of five children's records and provider interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to each child's attendance or within 30 days after the first day of attendance. Evidence: 1. According to the records provided, children 1 and 2 began attending on 8/22/2016. There was no documentation of physical examinations for children 1 and 2. 2. According to the record provided, child 3 began attending 3/1/217. There was no documentation of a physical examination. 3. According to the record provided, child 5 began attending 1/16/2017. There was no documentation of a physical examination. 4. The provider was unable to provide documentation of physical examinations for the above children.

Plan of Correction: The provider responded with the following: All parents will be required to obtain documentation of a physical examination within 30 days of the child's attendance.
