Technical Assistance:
Reviewed all upcoming changes to required background checks.

Comments:
An unannounced full renewal inspection was conducted today. The provider and two assistant caregivers were caring for 12 children, totaling 32 points. The children were observed playing with toys, coloring pictures, and interacting with each caregiver. The child care area continues to be clean and well-equipped. The children were to be served chicken, rice, mixed vegetables, apples, and milk for lunch later today. This inspection was completed between approximately 10:15 am and 11:40 am. Violations:
Standard #: 22VAC40-111-800-A
Description: Based on a review of administrative documents, it was determined that the provider does not have all required emergency preparedness documentation. Evidence: The provider does not have a complete emergency preparedness plan.

Plan of Correction: I will complete the plan this week and train my assistants on the information.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #4 does not include the mother's work address and phone number.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver training records, it was determined that two caregivers did not attend at least 16 hours of training courses during the past year. Evidence: The provider and caregiver B attended a total of only 6 hours of training courses during the past year.

Plan of Correction: I will make sure all of us attend at least 16 hours of training this year.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of caregiver background checks, it was determined that one repeat background check has not been completed. Evidence: The Criminal History Record Check (CRC) for caregiver B expired on 4/22/17.

Plan of Correction: I submitted the CRC request, but I have misplaced the findings paperwork. If I can't find the findings paperwork, I will submit another CRC request and place the findings report in her record when received.
