Comments:
An unannounced renewal inspection was conducted on June 2nd 2017 between 11:10am and 12:05pm. There were 4 children in the direct care of a provider and an assistant. Children were observed reading books, being fed, and playing with stuffed toys. Staff were observed engaging with each child throughout the inspection. A sampling of children's records, the new assistant record, evacuation drills, attendance records, the physical space, and supervision were observed and inspected. Description: Based on record review, 1 of 1 records did not have documentation of an orientation was not signed and dated by both the provider and the assistant.

Plan of Correction: I will print it out and have her sign it.
