Technical Assistance:

An unannounced monitoring inspection was made on 6/9/17 at approximately 10:00 a.m. and concluded at 11:45 a. m. The inspector met with the provider and Family Day Home standards were reviewed. There were 10 children present with points equaling 22 with the provider and one assistant present. The children were observed having free play with age and stage appropriate toys and watching a video. One observation was conducted. Conversations were held with the provider and children. No medications are being administered. The last evacuation drill was documented on 5/30/17. Shelter-in-place drill was conducted on 4/12/17. Five children records were reviewed. Provider and assistant record reviewed.

Standard #: 22VAC40-111-920
Description: Based on observation the family day home did not have a current menu. Evidence: On 6/9/2017, there was no menu posted for June 2017.

Plan of Correction: I will plan my menu by the month and post from now on.

Standard #: 22VAC40-111-60-A
Description: Based on review of children records the family day home did not have all required information on file. Evidence: On 6/9/17, after review of five children records, child #1 record did not include an emergency address.

Plan of Correction: I will have the parent complete this at pick up today.
