Standard #: VENDHOM-000-006-B Description: Based on review of children records the family day home did not have all required information on file. Evidence: On 6/9/17, child #1 did not have a written allergy plan for a diagnosed food allergy. Plan of Correction: I will ask parent to get an allergy plan from the doctor for child #1 at pick up today.
Standard #: VENDHOM-000-012-D Description: Based on review of staff orientation the family day home did not have all required components. Evidence: On 6/9/17, caregiver #1 did not have orientation in the areas of playground safety, supervision, lost/missing children, ill/injured children, medication administration, emergency preparedness plans, shaken baby, safe sleep, and food allergies. Plan of Correction: Will provide orientation today.
Standard #: VENDHOM-000-012-E Description: Based on discussion with the provider all caregivers do not have required training. Evidence: On 6/9/17, caregiver #1 did not have First Aid and CPR training. Plan of Correction: I will call today to schedule First Aid and CPR training for caregiver #1.

Evidence: On 6/9/17, the emergency plan did not include procedures for a lock down or reunification. Plan of Correction: I will work on this today.
