An unannounced monitoring inspection was conducted from 2:34pm through 4pm. There were five children present at the onset of the inspection. Children ranged in age from 11months to 2years old. Total points present equaled 11 points. Children were observed in play activities, snack, restroom routines, hand washing and one diaper change. A violation regarding household background checks was observed and appears on the violation notice.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review and interview the provider failed to ensure that persons living in the home of a licensed family day home provider has a of central registry record check within 30 days of a person beginning to reside in the home. Evidence: 1- There was not a central registry record check available for review during the inspection for household member 1. 2 - The provider stated that she could not locate the document. Plan of Correction: Going forward the provider will ensure that background checks for household members are maintained as required. A new central registry background check will be requested to replace the document that could not be located during the inspection.
