An unannounced monitoring inspection was conducted today. There are 11 children present (27 points) with 3 adults. 3 children's files were reviewed. See violation notice for missing information. Children were observed playing with a variety of toys and games during today's inspection. The newly installed play equipment must have resilient surfacing in the fall zones for swing and slides. Your application for renewal has been received. I will return to conduct your renewal study, prior to your license expiring. Standard #: 22VAC40-111-940-A Description: Based upon direct observation, the refrigerator temperature is not maintained at 40 degrees or below. Evidence: The thermometer in the refrigerator is registering 42 degrees. Plan of Correction: The temperature was lowered during the inspection to 40 degrees.
Standard #: 22VAC40-111-960-E Description: Based upon direct observation, bottles are labeled with the child's name, but are not dated. Plan of Correction: I will date the bottles.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, one child's file does not contain documentation of the parent's having received all required policies or procedures. Evidence: Child A's file does not contain documentation of the termination policy. Plan of Correction: I will complete the section and have the parent initial and date the change.
Standard #: 22VAC40-111-500-C Description: Based upon direct observation, the stationary play equipment is installed on a hard surface. Evidence: The newly installed play equipment does not have resilient surfacing under the swings and slides. Plan of Correction: I will purchase the mats to go under the swings.
