54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced monitoring inspection was conducted today from 2:00pm-3:30pm. There were 4 children(12 points) directly supervised by 1 staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There was one medication s on site today. Children were observed playing freely and napping. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. If you have any questions regarding this inspection, please contact the Licensing Inspector.

Violations:
Standard #: 22VAC40-111-700-A
Description: Based on medication review, prescription medication at the family day home was not kept in accordance with the family day home's medication policies. Evidence: 1. The family day home's policy is that no medication will be given to children by the provider. 2. Their was no parental or physician authorization for the emergency medication on site today. 3. The emergency medication on site today has an expiration date of June 2017.

Plan of Correction: The medication will be sent home today.

Standard #: 22VAC40-111-710
Description: Based on medication review, a child's emergency medication did not contain a prescription label with the child's name or prescriber's instructions pertaining to dosage, frequency and manner of administration. Evidence: Two doses of pre measured auto-injector of epinephrine were provided to the inspector without a prescription label or manufacturers instructions.

Plan of Correction: The medication will be returned to the parent today.

Standard #: 22VAC40-111-220-A
Description: Based on record review, caregivers who had agreed to amenities medication had not satisfactorily completed a training program for this purpose developed or approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist.

Plan of Correction: The medication will be sent home today. In the future the provider will not accept medication.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, one adult living in the home did not have a central registry check before three years since the date of the last central registry check. Evidence: Adult #2's last central registry check was completed on 9/11/2011.

Plan of Correction: The provider will mail out a new background check.
