Technical Assistance:
Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. VA Preservice training can be counted as annual training one time. *Please notify your inspector of termination/suspension of services for more than 24 hours as a result of an emergency. *Please notify your inspector ?preferably in writing ? of any serious injuries sustained at the facility (found in definitions in vendor agreement). Please label all substitute containers containing hazardous substances with the contents (for example, bleach and water in a spray bottle).

An unannounced subsidy health and safety inspection (SHSI) was conducted with the provider. A licensing inspection was also conducted on this date (see separate report for details). There are currently 3 assistants working at the FDH. The providers need to take the mandatory VA Preservice Training. All staff are current in first aid and CPR certification . There are no children with diagnosed food allergies currently enrolled ? please review the applicable standards regarding food allergies if necessary. The emergency preparedness plan was reviewed for reunification plans, home containment procedures, and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children from a security threat; shelter in place drills are to protect children from a natural or manmade threat. Description: Based on staff interview and review of files, the facility failed to complete Virginia Pre-service Training for Child Care Staff sponsored by DSS. Evidence: the provider has not completed Virginia Pre-service Training for Child Care Staff.

Plan of Correction: Provider and assistants will take the Virginia Pre-service training online.

Standard #: VENDHOM-000-029-A
Description: Based on review of the emergency preparedness plan, the provider failed to include caregiver responsibility and home readiness with respect to lockdown and reunification procedures. Evidence: there are no procedures documented for lockdown, home containment, and reunification procedures.

Plan of Correction: Provider will develop procedures for lockdown, home containment, and reunification.
