54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted with provider and 3 assistants. There were 12 children present, ranging in age from 2 months to 5 years, totalling 33 points. The children were playing with age appropriate toys, having lunch, and laid down for rest. Lunch served today: chicken nuggets, green beans, apple slices or apple sauce, and milk. Twelve children's files were reviewed and five household/staff files were reviewed. Please see Supplemental Summary for missing items. The immunizations for 9 pets were reviewed (one pet is too young to have yet received the rabies vaccination). All staff are current in first aid and CPR. There are no children receiving medication at day care - Medication Administration Training is required to administer medication to day care children. Standard #: 22VAC40-111-100-A Description: Based on review of twelve children's files, the provider failed to have documentation of a physical in each child's file within 30 days of attendance. Evidence: Child 1, Child 2, Child 5,, Child 8, Child 9, and Child 12 did not have documentation of a physical on file within 30 days of first attendance. Plan of Correction: Provider will ask the parents to provide the physical to day care.
Standard #: 22VAC40-111-70-A Description: Based on review of twelve children's files, the provider failed to provide all information in writing to parents before the first day of attendance. Evidence: Child 5, Child 6, Child 7, and Child 8 did not have signed documentation of having received the provider's "Policy on the Administration of Medication". Plan of Correction: Provider will have the parents sign page 2 of the "Policy on the Administration of Medication".
Standard #: 22VAC40-111-90-C Description: Based on review of 12 children's records, the provider failed to have documentation of an immunization for each child before first day of attendance. Evidence: Child 12 did not have documentation of a physical or well baby exam. Plan of Correction: Provider will ask parents to provide it again.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of five household/staff files, the provider failed to have documentation of an original criminal history record report for each staff within 30 days of employment. Evidence: Staff B (date of hire 8/31/2016) and Staff C (date of hire 3/13/2017)did not have documentation of an original criminal history record on file. Plan of Correction: The criminal history record checks have been mailed to Va. State Police last month.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of five household/staff files, the provider failed to have documentation of an original search of the central registry (CPS check) for each employee within 30 days of employment/residence. Evidence: Staff B (date of hire 8/31/2016) did not have an original search of the central registry on file. Plan of Correction: Provider mailed the search of the central registry CPS check to VDSS last month. It has not yet been returned.
