Technical Assistance:
An exit meeting was completed by the inspector with the provider to review the findings of the subsidy inspection. The provider was recommended to review all subsidy requirements.

Comments:
The licensing inspector conducted a subsidy health and safety inspection on Friday, June 16, 2017 from approximately 8:50 am to 11:30 am to determine the provider's compliance with the subsidy requirements. The licensing inspector reviewed the necessary documents, and inspected the overall physical plant due to provider's relocation. There are currently no children in care at the family day home. The inspector observed the required postings to include license, and prior inspection summary. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills, emergency preparedness plan to include the lockdown procedures were inspected. The emergency preparedness plan was last updated on 06/01/17. During this inspection, the provider's, and the household member's records were reviewed for compliance of standards. Standard #: VENDHOM-000-012-C
Description: Based on review of caregiver's record, the caregiver did not complete Virginia Pre-Service Training within the first 90 days of vendor agreement completion. Evidence: A review of the caregiver's record was completed today and there was no certificate of completion for the Pre-service Training. During a interview with the caregiver it was confirmed that the Pre-service training has not yet been completed.

Plan of Correction: Provider will call subsidy today to ensure they have updated email information. I will start training today, 6/16/17 until completion.
