54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced initial day inspection was conducted today. Observed 5 children (16-points) directly supervised by the provider. Ratio and supervision were in compliance. A sample of 2 children's records, and 2 caregiver records were reviewed. The physical plant and emergency procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: free play, parent drop off and practicing sharing toys. The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-60-B Description: Based upon record review, required information was not found in child(rens) records. Evidence: The records for child A and Child B were both missing medical insurance policy information and authorization for emergency medical care. Plan of Correction: Provider will begin using the model dss child enrollment form that includes all missing criteria in the childrens records. The provider is passing out enrollment forms tonight at parent pick up.
Standard #: 22VAC40-111-80-C Description: Based upon record review, child records were found to be missing required documentation. Evidence: Records for Child A and B were missing documentation of the provider reviewing proof of age and identity by either viewing a notification of birth letter,birth certificate or other approved documentation. Plan of Correction: Provider was given a model DSS child enrollment form which contains a section that allows for required documentation. Provider is passing out enrollment for to all families tonight.
