Comments:
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 6 children, totaling 12 points. The children were observed attending bible study class, completing academic work, and interacting with each caregiver. The children were to be served pizza, mixed fruit, and milk for lunch later today. This inspection was completed between approximately 10:45 am and 12:00 pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on a review of emergency documentation, it was determined that the provider has not conducted a monthly fire evacuation drill. Evidence: The provider did not conduct a fire evacuation drill during the months of April and May.

Plan of Correction: I will remember to do a fire drill every month.

Standard #: 22VAC40-111-120-B
Description: Based on a review of one caregiver record, it was determined that this record was not complete. Evidence: The file for the assistant caregiver does not include: verification of her age; emergency contact information; orientation training; a signed Sworn Disclosure Statement; a completed Criminal Record Check; a completed Child Protective Services Registry check.

Plan of Correction: I will have her fill out the cover sheet. I will conduct the reference checks and submit the background check requests. I will do her orientation training with her.

Standard #: 22VAC40-111-50-A
Description: Based on a review of documents, it was determined that the provider has not kept a written record of attendance each day this week. Evidence: The provider has not documented the children's attendance this week.

Plan of Correction: I will remember to do the attendance each day.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #1 and #2 do not include documentation showing that the provider viewed each child's original proof of identification.

Plan of Correction: I saw each child's original passport but I didn't document it properly. I will document it correctly in each child's record.

Standard #: 22VAC40-111-690-D
Description: Based on observations, it was determined that the provider is not using the required cleaning solutions when cleaning the diaper changing pad. Evidence: The provider uses cleaning wipes for the changing pad.

Plan of Correction: I will begin using a soap/water solution and a bleach/water solution.
