Comments:
An unannounced monitoring inspection was completed on June 23, 2017. The purpose of this visit was to measure the facility?s compliance with most, but not all, of the licensing standards. There were 4 children in care, for a total of 4 points with one caregiver. The children were observed playing board games and eating a snack of graham crackers and milk. The facility administers medication, but does not have any medication on site at this time. Caregivers have MAT training. The facility does not provide transportation. The last evacuation drill was conducted 6/15/17. The most recent shelter -in-place was conducted 4/14/17. Two caregiver, one household member and four children?s records were reviewed. Approximate time on site: 9:50am to 12:30pm

Standard #: 22VAC40-111-180-A
Description: Based on a review of records, the facility did not ensure that documentation of updated tuberculosis screening was obtained. EVIDENCE: The record of caregiver #2 contained a tuberculosis screening that was dated 9/5/14. The record of household member #1 contained a TB screening that was dated 10/24/14. There was no record of a TB screening that was conducted within 2 years of the most recent screening.

Plan of Correction: We will do new tuberculosis screenings to submit and will be done as soon as possible.
