Technical Assistance:
At this time there is no crib in the daycare. Because of this, your License cannot be issued for younger than 12 months. We can modify your license once you get a crib in the daycare. No medication can be given without first getting Medication Administration Training MAT

Comments:
An Initial Licensing Inspection was conducted with the provider beginning at 9:15 and ending at 12:00. There were 3 children in care, all preschool age. Children were observed playing with toys, having snack and chatting with the provider. Inspected all three levels of the home. Reviewed files and policies. There is sufficient space and supplies for 12 children. Family Background checks and TB tests are complete. Standard #: 22VAC40-111-700-A
Description: Based on observation, it was determined that the provider's medication policy is not correct. Evidence: The Policy on Mediation administration states that the provider will give non prescription medication . The provider is not certified to give any medication. MAT

Plan of Correction: The provider will change her written policy and notify all parents.

Standard #: 22VAC40-111-760-A
Description: Based on interview with the provider and observation, not all required emergency supplies were in the home. Evidence: There is no weather band radio in the home.

Plan of Correction: The provider will purchase a weather band radio.

Standard #: 22VAC40-111-760-A-1
Description: Based on observation, it was determined that the First Aid kit, did not have all required components. Evidence: There were no tweezers or triangular bandages in the first aid kit.

Plan of Correction: The provider will get all supplies needed.

Standard #: 22VAC40-111-790
Description: Based on observation, it was determined that not all numbers were posted Evidence: The provider does n0t have 911 posted.

Plan of Correction: The provider will post 911 next to the Poison Control Number.

Standard #: 22VAC40-111-800-A
Description: Based on interview with the provider it was determined that there was no written emergency preparedness plan and no emergency preparedness plan form given to parents. Also not evacuation Map posted. Evidence: The provider has recently moved and has not done the Emergency Preparedness plan

Plan of Correction: The Provider will complete the Emergency Preparedness Plan, the Map and the Information for Parents.

Standard #: 22VAC40-111-60-B
Description: Based on a review of three children's files, it was determined that not all files have all required information. Evidence: The file for Child #1 is missing the address for the second emergency contact and a physical examination. The file for Children #2 and #3 are missing a second emergency contact. All children are missing the Information and Agreement form

Plan of Correction: The provider will get from the parents.

Standard #: 22VAC40-111-330-A
Description: Based on observation, it was determined that the land line phones to not meet the requirements for phones that are not dependent on electricity. Evidence: All phones are cordless and require the base to be plugged into an electrical outlet.

Plan of Correction: The provider will get an "old fashioned" phone.

Standard #: 22VAC40-111-430-B
Description: Based on observation the gate at the bottom of the stairs to the Daycare area. does not meet the gate regulations to be attached to the wall. Evidence. The gate at the bottom of the stairs is pressure mounted and not permanently attached.

Plan of Correction: The provider will get a gate that will be attached to the wall.
