An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 9 points. The children were observed watching television, playing with toys, interacting with each caregiver, and eating lunch. The children were served chicken nuggets, french fries, bananas, and milk for lunch today. This inspection was completed between approximately 11:45 am and 1:00 pm. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include: documentation showing that the provider viewed the child's original proof of identification; a shot record; the full names for two emergency contact people. 2. Child record #2 does not include a second emergency contact person who is not a parent. Plan of Correction: Both parents will be asked for the missing information.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, it was determined that the provider's repeat TB Health Screening has expired. Evidence: The provider's TB Health Screening expired on 12/2/16. Plan of Correction: I will either find the documentation of my current TB test, or I will have a new TB test done.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that one item has not been maintained in a clean, operable condition. Evidence: The diaper changing pad is torn in various areas. The provider has covered one of the large torn areas with tape. Plan of Correction: I covered it with tape temporarily. I will buy a new changing pad.
