Technical Assistance:
Discussed sight and sound supervision and label the children's cups.

Comments:
Four children were in care during the inspection, total of eight points. The age range of the children was 23 months to six years old. Children were observed during free play and snack time. Inspection time 2:30pm-4:35pm.

Violations:
Standard #: 22VAC40-111-770
Description: Based on review the emergency flash light and radio at time were not working during the inspection. Evidence: There were no batteries for the flash light or radio.

Plan of Correction: I will have extra batteries.

Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records not all were complete. Evidence: Child A with a start date of 3/1/16 was missing the street address for two emergency contacts and the physician phone number. Child B with a start day of 7/5/17 was missing the work address and phone number for mother's work.

Plan of Correction: I will ask the parents for the missing information.

Standard #: 22VAC40-111-210-B
Description: Based on review of the 16 hours of annual training not all hours were completed. Evidence: The assistant did not have any documentation of annual training.

Plan of Correction: We will complete all the training hours and make up any missing hours from the past year.

Standard #: 22VAC40-111-680-B
Description: Based on review children's hands were not washed with soap and water after diaper changes. Evidence: One child's hands were not washed with soap and water after a diaper change.

Plan of Correction: I will wash the children's hands.

Standard #: 22VAC40-111-690-D
Description: Based on observation the diapering surfaces was not sanitized. Evidence: The provider did not sanitized the diaper pad after a diaper change.

Plan of Correction: I will clean and sanitize the diaper pad.
