Description: Based on observation and interview, the vendor did not post a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence - 1. The Provider stated (during an interview conducted on the date of inspection - 07/05/2017) that 1 enrolled child had a food allergy. 2. After observing all things posted within the Family Day Home, an allergy list was not available.

Plan of Correction: An allergy list shall be composed and posted.

Description: Based on review, the vendor does not have Lockdown procedures in place for the Family Day Home (FDH). Evidence - 1. On the date of inspection (07/05/2017), according to the FDH's Emergency Preparedness & Response Plan (EP&RP) the Lockdown procedures were not included. 2. The EP&RP did contain the evacuation, shelter-in-place, and relocation procedures.

Plan of Correction: Lockdown procedures shall be added to the FDH EP&RP form.
