Standard #: 22VAC40-111-230-A Description: Based on interviews and record review, the Provider did not obtain one certificate to verify assistant's training that was on May 6, 2017. Evidence: 1. Assistant's file has training certificates but shows assistant is short 4 hours in annual training. 2. The Provider has a certificate for May 6 and it consisted of 7 hours in development. She explained that they take trainings together and thought she had the document on file. Plan of Correction: I will find the certificate and place it in the file.
Standard #: 22VAC40-111-320-A Description: Based on observation, the Provider did not unplug a small device that was not in use. The Provider has a scented wax burner and it was unplugged and not in use. Plan of Correction: I will remove it since I don't use it.
