Technical Assistance:
Discussed upcoming changes to the background check requirements. Providers are not to begin the fingerprinting, but to continue meeting the current requirements. Memos regarding changes to the background check regulations will be sent with updates.

Comments:
An unannounced monitoring inspection was conducted today from 10:45am-1:00pm. There were 8 children (21 points) directly supervised by the provider and two assistant caregivers. Children were observed eating a nutritious lunch (pasta, tomato sauce, vegetables, strawberries, and milk), watching an educational video, and playing restaurant. Diapering and hand washing procedures were also observed. Infants were observed being held and listening to music while in bouncy seats. The home, 3 staff records, 7 child records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today and the provider does not administer medication (only topical ointments and sunscreens). There was an abundant supply of books, toys, and learning materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of a child's attendance. Evidence: Child #1 (start date 6/12/17) did not have documentation of a completed physical examination.

Plan of Correction: I will follow up with the parent today. She will fax or email me the paperwork.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not made inaccessible to children. Evidence: Clorox Toilet Cleaner and a bathroom cleaner were found in an unlocked cabinet in the bathroom.

Plan of Correction: Corrected at time of inspection.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, 3 of 4 caregiver and household members did not have repeat central registry and criminal history record checks before three years since the most recent reports. Evidence: 1. Caregiver #1 and #2 had criminal history record checks expired on 2/10/17 and central registry findings expired on 2/7/14. 2. Household member #1 had a criminal history record check expired on 2/11/17 and central registry findings expired on 2/7/14

Plan of Correction: I will send them next week (and have them notarized over the weekend). The reports have changed and I needed updated forms from them.
