An annual subsidy health and safety inspection (SHSI) was conducted from 10:50 a.m. to 1:00 p.m. with the provider and assistant. Three staff records were reviewed including staff training and CPR and first aid information. SHSI requirements were reviewed. Standard #: VENDHOM-000-012-A Description: Based on review of documentation, not all staff had Virginia Pre-Service Training. Staff #2 and Staff #3 did not have Virginia Pre-Service Training as required. Plan of Correction: Provider has discussed VA Pre-Service Training with Staff. Standard #: VENDHOM-000-029-B-3 Description: Based on review of documentation, the family day home did not have evacuation reunification in the Emergency Plan. Evidence: The Emergency Preparedness plan did not include procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Plan of Correction: Provider shall embody evacuation reunification process in the plan and address the procedures to children their parents and representatives.

Evidence: The Emergency Preparedness plan did not include procedures to address lockdown procedures. Plan of Correction: Provider shall provide lockdown procedure in the emergency plan and explain the process to children parents and authorized representatives.
