54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today. Observed 7 children (21 points) directly supervised by 2 caregivers. A sample of 2 children's records, 2 caregiver records, and 1 household member record were reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, rest time, blocks, and infant care. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted from approximately 10:45am to 12:15pm. Standard #: 22VAC40-111-830-A Description: Based upon documentation reviewed, emergency evacuation procedures were not practiced monthly. Evidence: An emergency evacuation drill was not done in June, 2017. Plan of Correction: I will make sure to practice drills monthly.
Standard #: 22VAC40-111-60-B Description: Based upon documentation reviewed, children's records were incomplete. Evidence: 1) One of the two children's records reviewed did not contain the father's work telephone number (Child A; Date of enrollment (DOE) 11/28/2016). 2) One of the two children's records reviewed did not contain the child's first date of attendance (Child B). 3) Two of the two children's records reviewed did not contain the name, address, and telephone number of a second emergency contact (Child A and B). 4) Two of the two children's records reviewed did not contain parent's signed acknowledgement of the receipt of required information, such as: check in / out procedures, fees, etc. (Child A and B). 5) One of the two children's records reviewed did not contain the policy number of the child's medical insurance (Child B). Plan of Correction: I will ask the parents for the missing information today during pick up.
Standard #: 22VAC40-111-540-A Description: Based upon observations made, a safety strap was not used and securely fastened when children were using infant swings. Evidence: There were two infants (Child A and B) in two different swings without a safety strap securely fastened. Plan of Correction: Corrected during today's inspection.
