Technical Assistance:
Technical assistance today included record keeping and annual updates; the intent of documentation of dated menus to include any changes in food; back-up providers; references; written verification of proof of identification. We also discussed the fabric for blankets and sheets in cribs; ages that children can use cots; labelling food from home with name and date; and the intent of documentation of attendance for emergency situations. An unannounced renewal inspection took place this morning from 10: 30 - 12:45 pm. There were ten children present with the provider and two assistants for 30 points. I observed children playing with manipulative games at the table; having a "tea party," eating lunch and preparation for naptime. No children's medications were reported on-site today. Standard #: 22VAC40-111-900-A
Description: During today's inspection, a child had access to a known choking food. (uncut grapes.) Evidence: Although an assistant was cutting grapes as required, the bowl of uncut grapes was observed on the high chair accessible to the child while staff were busy with serving lunch.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-920
Description: Based on a review of the posted snack menu during today's inspection, the menu did not meet all administrative requirements.Evidence: The menu was not dated as required; and substitutions today (cucumbers instead of carrots) were written on a marker board at lunchtime, not listed in a way where they were available to parents before the food was eaten; and in a way they could be kept on file as required.

Plan of Correction: I have the same menu every week. I will list possible substitutions in advance. on the bottom of the menu.

Description: Based on a review of staff records today, not all required information was available today. Evidence: Assistant # 1 and # 2 did not have written documentation of their emergency contact information and documentation of education and experience.

Plan of Correction: I will print out the page for staff and they will complete it today.

Standard #: 22VAC40-111-50-A
Description: Based on a review of records, not all children present were listed on attendance today within the required time. Evidence: A check of the written record of attendance at 10:30 indicated one child present was not listed as required.

Plan of Correction: We will write down the children's names as they arrive.

Standard #: 22VAC40-111-60-B
Description: Based on a review of a sample of children's records today, not all required information was present. Evidence: There was incomplete emergency contact information and physician's information for Child #4. Although there were parents; signatures, there was no documentation indicating the presence or absence of liability insurance as required (boxes were not checked) for Child #2 and #4. There was not consistent annual updates/reviews documented of children's information as required. In two files, information was reviewed in 2014 & 2017.

Plan of Correction: I will make sure the parents fill out the entire forms but two of those children are leaving. The more recent liability form will be used, and I will go back and have some parents sign it again. We will do updates for everyone in September and more often if they have been here over a year before September.

Standard #: 22VAC40-111-170-A
Description: Based on a review of staff records today, there tuberculosis screenings present for two individuals did not document an absence of TB as required. Evidence: Although the paperwork stated that Assistant #2 had a TB screening done in 3/17, and Household Member # 1 had a screening 7/17, there was no documentation of the results of those screenings as required.

Plan of Correction: They will bring in the papers with the results from the doctor.

Standard #: 22VAC40-111-690-E
Description: During today's inspection, soiled disposable items were not disposed of in a working foot-operated trash as required. Evidence: While changing diapers, the available step-on trash was not working, leaving the assistant to use hands to open the lid.

Plan of Correction: It will be replaced.
