An unannounced monitoring inspection was completed on 7/19/17. There were six children present arriving from a field trip with the provider and an assistant. The point total was 15. One child departed and two other children arrived which increased the point total to 20. Five newly enrolled children's records and two previously reviewed children's records were reviewed. The inspector arrived at the provider's home at 12:00 pm and departed at 3:00 pm. The provider was not at home when the inspector arrived but arrived shortly after from a field trip. The children were observed during arrival from a field trip, during lunch, during nap, and during arrival/departure. There was discussion with the provider about the new background check requirements.

Description: Based on record review, the provider failed to ensure that three of five newly enrolled children's records were complete with all required information. Evidence: 1. The record of child #4 was missing the policy number for the child's medical insurance. 2. The record of child #6 was missing documentation of previous child care, if any. 3. The record of child #7 was missing the father's work phone number (the space to document this was left blank and there was a work place documented). Child #7 was missing documentation of previous child care, if any.

Plan of Correction: The record of child #7 was corrected during the inspection when the child was picked up. The provider called the parent of child #4 during the inspection to get the policy number for the child's medical insurance which was documented during the inspection. The provider will have previous care, if any, documented for child #6.
