Technical Assistance:
Loudoun County will continue to provide trainings for providers on Saturdays. Each class is $25. Discussion on the importance of fire evacuation drills and the routine for children to understand and follow the fire evacuation procedures. Provider will need to pursue the special zoning process with Loudoun County. Until this process is completed, the licensed capacity will be reduced from 12 children to 9 children. Please forward a copy of your new zoning permit once obtained.

Met with provider today to complete an unannounced Renewal inspection. There were a total of 10 children (31points) with Provider and 1 assistant, within supervision guidelines. The children were observed actively playing with the equipment and toys both individually and in small groups. Children were working puzzles and matching card games. Provider transitioned the children from one activity to another to ensure that all children remained stimulated and actively engaged. Infants were observed playing and being held by staff. Diapering and hand washing procedures were observed. The home was found well maintained, organized and clean. Ample toys and equipment available and accessible to the children in care today. Outside baby pool observed, discussed the height of the water when children are in the pool. Positive interactions observed today. Some violations found today pertaining to recordation and emergency evacuation drills. Corrective actions were provided and will be evaluated at the next licensing inspection. Description: Based on provider documentation, monthly evacuation drills were not completed. Evidence: A review of the provider's evacuation drills was completed, provider has not completed a monthly evacuation drill since December 2016.

Plan of Correction: I will complete a evacuation drill this week.

Standard #: 22VAC40-111-830-B
Description: Based on a review of the emergency drill documentation, shelter in place drills were not completed. Evidence: Provider has not practiced the shelter in place procedures twice during the past year.

Plan of Correction: I will complete a shelter in place drill this week.

Standard #: 22VAC40-111-70-A
Description: Based on child record review, information was not provided in writing to one child's parent. Evidence: Child #1 began care on 4/17/17, the provider's policy on the provision of food, pet disclosure and DSS public web site .

Plan of Correction: The form for this information was in the child's record but the parent did not sign having received. Will have parent sign this document today.

Standard #: 22VAC40-111-80-A
Description: Based on child record review, documentation of proof of identity was not obtained. Evidence: Child #2 began care 12/2016, no documentation of proof of identity was obtained.

Plan of Correction: Will obtain documentation from the parent this week.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunizations were not obtained prior to a child's enrollment. Evidence: Child #2 began care on 12/12/16, immunizations were received on 1/17/17. Child #3 began care on 3/1/17. Immunizations were received on 3/16/17.

Plan of Correction: Provider will ensure that immunizations are obtained before the child begins care.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on staff record review, a background check was not obtained within 30 days of employment. Evidence: Staff #2 began employment in December 2016, no central registry search results maintained for this staff.

Plan of Correction: The central registry search request was sent, but the search was returned because staff does not have a middle name and the request was determined incomplete. Provider will call the CPS and request what to do to obtain a search without a middle name.
