An unannounced renewal study was conducted today. There are 9 children present including one of the provider's own (16 points) with 3 adults. 4 adult and 8 children's files were reviewed. See violation notice for missing information. Children were observed washing hands, eating lunch, playing with toys and resting quietly. Standard #: 22VAC40-111-800-A Description: Based upon a review of the emergency preparedness plan, the written plan is inconsistent with the "Provisions of the emergency preparedness plan" notification given to the parents. Evidence: The written plan's relocation address is not the same as the provision given to the parents. Two different locations are listed. Plan of Correction: I will update the written plan to include the library address.
Standard #: 22VAC40-111-120-B Description: Based upon a review of files, the assistant's file is incomplete. Evidence: Missing information includes but may not be limited to: 2 references prior to hiring, documentation of education and experience, address, emergency contact information, date of employment. Plan of Correction: I will contact reference today. I will have the Asst./Sub record completed today. Orientation was conducted, just not documented. It will be done today.

Standard #: 22VAC40-111-60-B Description: Based upon a review of files, children's files are incomplete. Evidence: Missing information includes: *1 of 7 files reviewed was missing allergy information (Child 1) Plan of Correction: I will ask the parent to complete the information.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, parents have not been provided in writing all required policies. Evidence: Child 7 is missing the termination policy. Plan of Correction: I will update the file today.
Standard #: 22VAC40-111-500-C Description: Based upon direct observation, one slide is over a hard surface. Evidence: The corkscrew slide does not have a resilient surfacing mat at the exit. Plan of Correction: I will order another mat. We will not use the slide until the mat is here.
Standard #: 22VAC40-191-40-D-1-b Description: Based upon a review of files, one adult file does not contain documentation of background checks past 30 days of employment. Evidence: Adult A's file is missing a complete Criminal Record Check (CRC) and a complete Central Registry Search (CPS). Plan of Correction: The forms were mailed off and I am waiting for the return. One form had to be submitted a second time. I will call today and follow upon the forms.
Standard #: 22VAC40-191-60-C-1 Description: Based upon a review of files, Adult A was allowed to work, past 30 days of employment without a completed CRC search on file. Plan of Correction: Adult A will not be allowed to work until the form is returned.
