54.1-3408 Provider must be MAT certified to administer prescription medication.

Discussion was held on the topics of current enrollment, current staffing, the upcoming finger print checks, new time table for other background checks, and Standard #750.C to treat all prescription topical skin products as regular prescription medication.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located near Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (18 points) with 2 caregivers. A total of 5 children?s files and all 4 staff/household member?s files were reviewed. The children were engaged in the following: working on an "invention project," games, toy play, story time, diaper changing, lunch, bathroom breaks, hand washing, and other organized activities. Standard #: 22VAC40-111-730-D Description: Based on observation, 2 of 2 medications were not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (08/03/2017), one medication for Child #10 stored in an upper kitchen cabinet did not have a safe locking method in place. 2. Another medication (a prescription topical skin ointment) for Child #4 was left in an open storage cup that sat on a shelf located in the sleeping room near the diapering table. Plan of Correction: A lock will be installed on this upper cabinet.
Standard #: 22VAC40-111-170-A Description: Based on review, 1 of 4 staff/household member's files reviewed indicated that the Provider did not obtain for each adult household member prior to coming into contact with children a current Report of Tuberculosis (TB) Screening form documenting the absence of tuberculosis in a communicable form. Evidence - 1. On the date of inspection (08/03/2017), Member #4 is now an adult living within the residence. 2. Initial TB documentation was not available for review. Plan of Correction: TB documentation shall be obtained. Member #4 is scheduled to move out of the home.
Standard #: 22VAC40-111-320-B Description: Based on observation, child-resistant protective covers larger than 1-1/4 inches in diameter were not installed on 1 of 7 unused electrical outlets accessible to children under five years of age. Evidence - 1. On date of inspection (8/3/2017), an electrical outlet located within the sleeping room, on the same was as the room door, placed half way up the wall was missing outlet covers. 2. At the time of observation, there were 8 preschooler in care. Plan of Correction: Outlet will be covered.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was absorbent. Evidence - 1. On the date of inspection (08/03/2017), a changing pad was observed on the diapering table that had a covering who's sides were absorbent. 2. During the inspection, an infant was observed being changed on this surface. Plan of Correction: The current pad will be changed to a non-absorbent covering.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review, 1 of 4 staff/household member's files reviewed indicated that it has been more than three years since the date of the most recent criminal history record check report (CRC). Evidence - 1. On the date of inspection (08/03/2017), the most recent CRC on file for Member #1 was dated - 11/04/2013. 2. This same documentation was previously cited on a monitoring inspection conducted on 01/26/2017. Plan of Correction: Will obtained a new criminal record check.
