Today's visit was a renewal inspection. The inspection began at 9:50am and concluded at 2:30pm. There were ten children in care including two of the provider's children and a total of twenty-one points. There were a total of three caregivers present today. During the inspection the outdoor play area was viewed, lunch time was observed, the entire home was viewed, nap time was viewed, three out of twelve children's records were reviewed, emergency supplies were viewed, both records for assistant's records were reviewed, the licensee's record was reviewed, and an adult household member's record was reviewed. The licensee stated their were no children enrolled with food allergies, dietary restrictions, or intolerances. New background check requirements were reviewed with the provider. Your renewal application has not been received in our office and once an application has been received and deemed complete a recommendation regarding your license can be made. Standard #: 22VAC40-111-90-C Description: Based on the inspector's review of three out of twelve children's records, the provider failed to ensure an immunization or exemption of immunizations was received before a child's first day of care. Evidence: Child #3 has a documented first day of care listed as 4/3/17 and the immunization record is dated 4/4/17 after the child began the attending the family day home. Plan of Correction: The provider will not allow a child to start care before an immunization record or exemption affidavit is received.
Standard #: 22VAC40-111-500-C Description: Based on the inspector's observations on the outdoor play area, the provider failed to ensure a piece of stationary play equipment was not installed over a hard surface. Evidence: The inspector viewed a stationary climber with a slide and three swings and the swings are over bare ground where the grass has worn away. Plan of Correction: Mulch will be added to the area under the swings.
Standard #: 22VAC40-111-500-D Description: Based on the inspector's observations on the outdoor play area, the provider failed to ensure "S" hooks for swings were tightly closed. Evidence: The inspector observed a total of five "S" hooks attached to swings on a piece of outdoor play equipment that were open more than the thickness of a dime or credit card. Evidence: The inspector viewed the outdoor play area and observed a new climber/slide/swing set. There were two tradiditinal swings along with a tire swing on the piece of equipment and a total of five open "S" hooks. Plan of Correction: The provider will look for a different style of hook or close the "s" hooks.
