An unannounced monitoring inspection was conducted on August 8, 2017 from approximately 9:10 am to 11:15 am. Provider and assistant were present with two children. Children were observed playing freely. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation and shelter-in-place drill documentation, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the provider during the exit interview conducted on this date.
Standard #: 22VAC40-111-90-A Description: Based on review, provider did not ensure 1 of 2 children's records has an immunization record prior to child beginning attendance. Evidence: On August 8, 2017, the record for child #1 (start date January 19, 2016) does not have an immunization record. Plan of Correction: Assistant will text the parent and ask her to bring in the immunization record.
Standard #: 22VAC40-111-210-A Description: Based on review, provider did not ensure 2 of 2 caregivers complete at least 16 hours of annual training. Evidence: On August 8, 2017, the following was found during record review: 1. The record for caregiver #1 has documentation of completing 11 hours of training in 2016. 2. The record for caregiver #2 has documentation of completing 8.5 hours of training in 2016. Plan of Correction: Caregivers will do training on the computer as well as there is a training scheduled in September.
