Technical Assistance:
Discussed the importance of correcting the repeated/uncorrected violations that were found during the last two inspections. CPR and First Aid certification must be maintained to ensure current certification. Please refer to the online training resources provided to you in March 2017 in order to comply with the required 16 hours annually. Discussion on documentation needed to verify proof of identity. Information provided to provider regarding the 10 Pre-Service training available.

Comments:
Met with provider to complete an unannounced monitoring inspection today. 4 children were present at the beginning of the inspection and two additional children arrived during the inspection which totaled 6 children (11 points) and provider, within supervision guidelines. Children were observed having snack which consisted of animal crackers, stocking oversized legos and interacting with provider. Observation of physical plant, record review and daily programming observed. Repeated/uncorrected violations were found today along with additional violations regarding posting requirements and emergency evacuation drills. Lunch planned today is corn dogs, carrots and grapes. Standard #: 22VAC40-111-830-A
Description: Based on review of the provider's emergency evacuation drill log, monthly drills were not completed. Evidence: The last documented emergency evacuation drill was dated March 2017.

Plan of Correction: I will complete a drill today.

Standard #: 22VAC40-111-100-A
Description: Based on child record review, documentation was not maintained. Evidence: Repeated and uncorrected since the November 2016 and March 2017 inspection, documentation of a physical exam was not maintained for Child #1 who began care on 6/13/2016.

Plan of Correction: I have requested the exam from the child's parent. Child had a physical last week and provider will obtain a copy of this exam.

Standard #: 22VAC40-111-60-A
Description: Based on child record review, three records were not maintained up to date. Evidence: Child #1's record does not have an acceptable form of proof of identity. Child #2, #3 and #4's emergency contacts were last reviewed on May 2016. An annual review is required.

Plan of Correction: I was not aware that a birth letter was not acceptable. I will verify proof of identity with the child's birth certificate and document. Annual review of emergency contacts will be completed today.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, additional immunizations were not obtained. Evidence: Documentation of additional immunizations for Child #4 was not obtained. Last immunizations maintained were dated 2008. This child is currently 9 years old.

Plan of Correction: I will request updated immunizations for this child.

Standard #: 22VAC40-111-210-A
Description: Based on staff record review and staff statements, annual training has not been obtained. Evidence: Repeated and uncorrected since the last licensing inspection in March 2017, Provider has not obtained the required 16 hours of training.

Plan of Correction: Provider will complete the 10 hours pre service training in addition to 6 additional hours by December 2017.

Standard #: 22VAC40-111-230-A
Description: Based on staff record review and staff statements, written training documentation was not maintained. Evidence: Repeated and uncorrected from the March 2017 inspection, no documentation found for completed CPR/First Aid certification.

Plan of Correction: Provider attempted to email instructor during the inspection. Provider will research online to see if new certificates can be requested. Provider stated that she completed both courses but has not received the certification cards.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of physical plant, required postings were not found. Evidence: The most current Violation Notice from the March 2017 was not posted as required.

Plan of Correction: This will be corrected by posting today's inspection.
