Today an unannounced renewal visit was conducted with the Provider this visit took place in the afternoon between 11:45am-2 there were 9 children with the Provider and 1 assistant for a total of 28 points within the supervision guidelines. The children were observed having dramatic play, playing with puzzles, listening to music, storytime, lunch and nap time. Lunch consisted of lentil soup and rice, apples and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B Description: Based on review of the records, the following information was missing from children records. Evidence: Child #1 record, did not include two emergency contact persons address and phone number, and medical insurance policy number. and parents employer phone number. Plan of Correction: Provider stated that she will request the information from the parent.
