Technical Assistance:
We discussed the requirements for physical examination records. The risk assessment for a violation was provided. We reviewed the requirements for a weekly menu. As a reminder, sixteen hours of training are required.

Thank you for your assistance during today's unannounced inspection, conducted in conjunction with a subsidy vendor inspection, from 12:05 PM to 4:55 PM. Today, there were eleven children in care, ages infancy through fourteen years (20 points) with two caregivers. Two other resident children over the age of eight years were present. I viewed activities, daily routines, staffing, supervision, interactions with children, equipment, diapering procedures, meals/snacks, five records for children, four records for caregivers and residents, caregiver training, emergency supplies and drill records, emergency planning, posted information, menu, hand washing routines, indoor and outdoor areas. The children did not play outside due to the rainy weather. They enjoyed the toys, drawing and a card game. The youngest children were provided comfort and needed attention. We discussed the need for one more parent notice verification per your plan that is due this week. Let me know if you need any assistance. 540/430-9259

Description: Based on a review of five records for children, and an interview with the provider, the physical examination records for child 2, 3 and 4 were not obtained within the first thirty days of attendance. Evidence: Child 2 began attending on June 6, 2017 and the physical examination report was completed on August 7, 2017. There were not any physical examination records in the files for child 3 and 4 and they started attending on June 13, 2017.

Plan of Correction: During pick-up the provider asked the parent for copies of the physical examination reports for child 3 and 4. The provider will follow-up with the parent during the week to make sure the reports are obtained for the records. In the future, new parents will be told the physical examination records must be provided in the first month of attendance.
