Licensing Inspector arrived at 9:30am and departed at 12:15pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were six children, ages two to three years old in care with the provider totaling 12 points. Children were observed watching children's programming and doing name, number, and letter recognition activities with the provider along with coloring.
Standard #: 22VAC40-111-110-B Description: Based on record review, in two of six children's records reviewed, the provider did not ensure that each medical report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department. Evidence: During the inspection conducted on August 22, 2017, the immunizations for Child #1 and the physical and immunizations for Child #3 had not been signed by a licensed physician, the physician's designee, or an official of a local health department. Plan of Correction: Provider will talk to the children's parents regarding the signatures.
Standard #: 22VAC40-111-640-1 Description: Based on observation and inspection of the family day home, the provider did not ensure that acts or threats thereof are forbidden in the following areas: Physical punishment including, but not limited to, striking a child, roughly handling or shaking a child, biting, pinching, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment. Evidence: During the inspection conducted on August 22, 2017, the licensing inspector heard the provider say to a child in care: "If you touch [my personal area] one more time, I am going to pop your hand and call your mom". Plan of Correction: Provider will not use that as behavior management.
