You can make a copy of the Publication Order form, check the boxes for Family Day Home Standards - $8.00 and the Forms Packet - $5.00. An unannounced monitoring form was conducted beginning at 9:10 and ending at 10:45. There were two children in care on this date. Children were observed playing with toys and interacting with the provider and family, Reviewed violations from previous inspection. See Violation Notice for those standards that are in violation. Demonstrated the web site to the provider and the location of needed forms. Provider is requesting a change in age range to Birth - 12 years. There is now a crib available. Standard #: 22VAC40-111-50-A Description: Based on observation, it was determined that the provider is not keeping daily attendance record. Evidence: The provider does not curretly have a form to keep attendance record. Plan of Correction: I will make a form and keep all attendance forms in a file folder after they are complete.
Standard #: 22VAC40-111-70-A Description: Based on interview with the provider and review of the children's files, it was determined that there is no Information for parents form for Parents for each enrolled child Evidence: The provider could not locate the form on line. Plan of Correction: The provider will order the Forms Packet on line and make copies.
