Comments:
An unannounced monitoring inspection was conducted today from 12:05pm-1:20pm. There were 4 children (8 points) directly supervised by 2 staff. The home, 2 staff records, 11 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and watching television. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review, the provider failed to ensure that emergency evacuation procedures are practiced monthly. Evidence: Reviewed the emergency evacuation procedure log and found that there was no evacuation drill practiced in July.

Plan of Correction: Will be completed by the end of the month

Standard #: 22VAC40-111-980-A
Description: Based on record review general written permission was not obtained from the parent of each child that the provider takes off the premises of the day home. Evidence: 9 out of 11 children were missing written permission from the parent to be transported off of the premises of the family day home

Plan of Correction: will be completed a week from today

Standard #: 22VAC40-111-240-A
Description: Based on observation an area in the home wasn't maintained in a safe condition. Evidence: 1) The corner of the sink in the bathroom located of the basement is broken and not safe for children.

Plan of Correction: Will change the entire counter by the end of next month

Standard #: 22VAC40-111-340-C
Description: Based on observation the bathroom did not contain liquid soap. Evidence: The bathroom in the basement did not contain liquid soap

Plan of Correction: Will be replaced today

Standard #: 22VAC40-111-350-E
Description: Based on observation the hot water tap was not within the range of 105?F to 120?F. Evidence: The bathroom located in the basement exceeded 120?F, the water was hot enough to burn the hand of the inspector.

Plan of Correction: will be completed by monday

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review two employees were missing a recent criminal history and and central registry check. Evidence: 1) Staff #1 - doesn't have an updated CPS form. The last one was completed 1/2/13 2) Staff #2 - doesn't have an updated CPS form. The last one was completed 1/14/ 2014; 3) Staff #2 - doesn't have an updated Criminal History check. The last one was complete 1/7/14.

Plan of Correction: CPS will be completed by the end of the month Criminal history will be completed by September
