Discussion was held on the topics of current enrollment, current staffing, and the upcoming new finger print checks.
The following inspection took place between the hours of 9:30 am through 11:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 6 children (14 points) with 2 caregivers, and 1 volunteer. A total of 10 children?s files and 7 staff/household member?s files were reviewed. The children were engaged in the following: large motor skill play outside on FDH playground, toy play inside, bathroom breaks, hand washing, music, infant sleeping, infant feeding, lunch, and other organized activities. Standard #: 22VAC40-111-780-B Description: Based on review, 2 of 10 children's files reviewed indicated that the Provider did not review annually with the parent the emergency contact information (required in 22 VAC 40-111-60 B 2) to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence - 1. On the date of inspection (08/22/2017), the most current documented review of the emergency contact information listed for Child #1 was dated - 06/07/2016. 2. The most current documented review of the emergency contact information listed for Child #3 was dated - 06/08/2016. Plan of Correction: The Parents of Child #1 and Child #3 shall be contacted, and the required review and signature shall be obtained.
