22VAC40-80 THE LICENSING PROCESS.
The licensing inspector and department representative attempted a monitoring inspection on Thursday, August 17, 2017 at approximately 10:10am to the address listed on the the current license issued by the department. During the attempted monitoring inspection, it was determined the provider had moved to a new residence. See violation notice. The licensing inspector informed the provider by phone that a modification request needed to be completed prior to relocating a family day home to a different location other than the one listed on the current license. A modification form was emailed to the provider on August 17, 2017. Please contact Lynn Powers at (804) 662-9790 if you have any questions.
Standard #: 22VAC40-111-30-A Description: Based on observation and interview, the provider did not follow terms listed on the current license issued by the department.
At approximately 10:10am on August 17, 2017, the licensing inspector attempted a monitoring inspection at the provider's home. Upon arriving at the home, a resident of the address listed on the current issued licensed stated the provider did not reside in the home. The licensing inspector called the provider on the phone. Over the phone, the provider stated that she had moved to a new residence "about three weeks ago"and was caring for three children at her new residence. A license is not transferable when there is a change of location.
Plan of Correction: Signed papers of modification and emailed them back to the licensing inspector to be monitored again at the new address.
