An unannounced monitoring inspection took place between the hours of 9:45 am through 12:00. There was 6 children (21 points) with 3 caregivers. A sample of 3 children's files and 4 staff/household member's files were reviewed. The physical plant, supplies and emergency procedures were reviewed. There were no medications on site. The home was clean, well organized and had age appropriate toys and equipment. The children were engaged in the following: toy play, coloring, and listening to music. Standard #: 22VAC40-111-120-B Description: Based on review and interview with the provider, one of the Assistant's records was incomplete. Evidence: 1. Assistant B's file did not contain the following: Emergency contact name, address and phone number, and date of employment. 2. The provider stated the form had been completed but could not find the documentation. Plan of Correction: This will be redone today.
Standard #: 22VAC40-111-200-C Description: Based on review of records and interview with the provider, the documentation of the orientation was missing from the Assistant's file. Evidence: 1. Assistant B's file was missing signed documentation of the orientation given by the provider. 2. The Provider stated the Assistant B started in March 2017 and was given the orientation training, but could not locate the signed documentation of the orientation. Plan of Correction: This will be redone today.
Standard #: 22VAC40-191-40-D-1-b Description: Based on review of four staff/household records and interview with the provider, documentation of the sworn statement for one Assistant was missing. Evidence: 1. Assistant B's file was missing documentation of the Sworn Statement prior to the first day. 2. The Provider stated the Assistant started in March 2017 and had signed the Sworn Statement prior to the first day, but could not locate the document. Plan of Correction: This will be redone today.
