Technical Assistance:
We discussed the requirements for TB screenings and the operable landline phone. The risk assessments for violations were provided.

Comments:
Thank you for your assistance during the announced inspection conducted from 2:30 PM to 3:00 PM for the purpose of modifying the license for the family day home operation at the new residence. There were four children in care along with one resident child, ages infancy to four years (12 points) with the provider. I viewed indoor and outdoor areas, posted information, equipment, attendance, staffing, supervision, interactions with children, pets, lunch/menu, diapering procedures, and emergency planning. The children had access to a variety of age appropriate toys. Two basement rooms had adequate space for the care of the children and there was easy access to the bathroom and hand sink. Issuance of a modified license in the provider's new name and address will be recommended. Let me know if you need any assistance. 540/430-9259

Violations:
Standard #: 22VAC40-111-170-A
Description: Based on a review of records for three caregiver/residents, and an interview with the provider, there was not a TB screening and statement for resident 2. Evidence: The model form for the TB statement for resident 2 did not include findings of a screening. The provider indicated that they waited after the move to inquire when the health department conducts TB screenings.

Plan of Correction: The caregiver contacted the health department and Medic Express about the availability of TB screenings. The updated TB screening and statement will be obtained for the record.

Standard #: 22VAC40-111-180-A
Description: Based on a review of three caregiver/resident records, and an interview with the provider, the TB screening and statement for caregiver 1 was not updated before two years from the date of the most recent statement. Evidence: The most recent TB screening was completed on July 8, 2015. The model form with the caregiver's name did not include findings of a more recent TB screening.

Plan of Correction: The provider contacted the health department and Medic Express about the availability of TB screenings. The TB screening and signed statement will be obtained and placed in the file. In the future, arrangements for updated TB screenings will be made in advance of the due date when it has been two years since the last screening.

Standard #: 22VAC40-111-280
Description: Based on observation of the cleaning supplies, there were cleaning agents accessible to children in the bathroom. Evidence: there was Lysol disinfectant spray, Scrubbing Bubbles foaming bleach spray, and Fabuloso cleaner on the sink counter. A gallon container of bleach was in the cabinet below the sink and with no locking mechanism.

Plan of Correction: A resident started installing locks to cabinets during the inspection. All cleaning supplies will be in locked areas or on high surfaces where children can't reach.

Standard #: 22VAC40-111-330-A
Description: Based on observation of the home telephone available, and an interview with the provider, there was not an operable landline telephone in the home. Evidence: When questioned for the landline phone number, the provider indicated that the line for the phone would be connected this week and a jack will be installed in the day care area of the basement. The phone that will be used was stored in the day care area. Only the provider's cell phone was observed and available for use during all day care hours.

Plan of Correction: The landline telephone number was looked up during the inspection and will be provided to parents. The arrangements for connection for the landline was made and expected this week when someone with the telephone company installs a new jack in the day care area of the home.
