Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 7 children, totaling 16 points. The children were observed assembling puzzles, playing with toys, attending Korean class, and interacting with each caregiver. The children were to be served fried rice with mixed vegetables, and milk for lunch later today. Caregiver records, administrative documents, and children's records were reviewed today. The provider reported there are no children's medications on site today. This inspection was completed between approximately 9:30 am and 10:45 am. Thank you for meeting with me today. If you have any questions regarding this inspection please email me at sandra.dimperio@dss.virginia.gov

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three children's records were not complete. Evidence: 1. Child record #2 does not include: a shot record, a signed "Declaration of Liability Insurance" document; a signed "Medication Policy" statement. 2. Child record #3 does not include: the information for a second emergency contact person; a physical exam report. 3. Child record #6 does not include: the address for each emergency contact person.

Plan of Correction: All parents will be asked for the missing information.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: One electrical outlet on the bottom half of the wall in the children's lunch room was not covered.

Plan of Correction: The outlet will be covered before the children return to that room for lunch.

Standard #: 22VAC40-111-690-D
Description: Based on interviews, it was determined that the provider has not started using an acceptable sanitizing solution after each diaper change. Evidence: The provider has not started using a mixed sanitizing solution for the diaper changing pad after each diaper change. The provider only uses a soap/water solution.

Plan of Correction: I misunderstood the requirement for soap/water and bleach/water. I will start using the bleach/water solution in addition to the soap/water solution.
