- Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, changes to background checks with regards to expiration dates, new sworn disclosure statements, and (if needed) to obtain CPS checks from other states as listed from the past five years. Discussion was also focused on the new location of the Family Day Home (FDH). The new space offers a larger backyard for the children to enjoy, and a more organized and spacious downstairs dedicated to the child care business. The FDH continues to offer a large amount of toys and equipment for the children in care to enjoy. The Provider expressed her frustration with the County of Loudoun and the need to obtain the appropriate documentation in order to complete the modification to her license; thus reflecting the change of address. The Provider was informed that until the modification is approved, the FDH is operating outside of the terms of the current issued license.
The following inspection took place between the hours of 9:30 am through # pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near the town of Waterford, Virginia. At the start of this unannounced monitoring inspection there were a total of 5 children (16 points) with 2 caregivers. A total of 7 children?s files and 4 staff/household member?s files were reviewed. Standard #: 22VAC40-111-790 Description: Based on observation and interview, the required emergency telephone numbers were not posted. Evidence - 1. On the date of inspection (09/08/2017), the following telephone numbers were not posted: 911, Poison Control, and emergency back-up staff who can take over and is less than 10 minutes away. 2. The Provider stated (during an interview conducted on the date of inspection) that the child care had just recently moved into the downstairs area and not everything had not yet been posted. Plan of Correction: Will post.
Standard #: 22VAC40-111-800-A Description: Based on interview, the Family Day Home (FDH) does not have a written Emergency Preparedness and Response Plan (EP&RP) specific to the new address. Evidence - 1. On the date of inspection (09/08/2017), the FDH currently does not have an EP&RP that addresses the new location and what procedures are to be implemented with regards to fire drills and shelter-in-place. 2. The Provider stated (during an interview conducted on the date of inspection) that a new EP&RP that reflects the new address has yet to be created. Plan of Correction: Will updated old EP&RP to reflect new location and share this documentation with the parents.
Standard #: 22VAC40-111-30-A Description: Based on interview and observation, the Provider did not ensure compliance with these standards and the terms of the current license issued by the department. Evidence - 1. On the date of inspection (09/08/2017), it was determined that the Family Day Home (FDH) is now operating outside the terms of its current license. 2. The current licensed was issued for the FDH to operate at 45 Pottersfield Drive in Lovettsville, Virginia. The Provider has since moved and is now operating her FDH at 15039 Coronet Place near Waterford, Virginia. The Provider stated (during an interview conducted on the date of inspection) that the FDH has been operating at this new address since June. Plan of Correction: The paperwork to submit a modification has been received. I am currently waiting for the Health Department to complete all testing with regards to the home (now a requirement in all rural areas when opening a FDH), and then the Contact with Local Zoning Administrator form. Once this documentation has been obtained it shall than be forwarded to the Division of Licensing.
Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 7 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (09/08/2017), a child's record was not available for review for Child #4. 2. This same child's record was cited during a previous inspection conducted on 03/13/2017 in which several items were missing from this file. Today's review was an attempt to determine through a follow up if the previously cited missing documentation/information was obtained. During the previous inspection conducted on 03/13/2017, the child was listed then as "Child #11." For today's inspection, the child is now identified as Child #4. Plan of Correction: The file shall be located or, if needed, an entire new file shall be completed; obtaining all required documentation and information once more.
Standard #: 22VAC40-111-90-A Description: Based on review, 1 of 7 children's file reviewed indicated that the Provider did not obtain documentation that a child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations). Evidence - 1. On the date of inspection (09/08/2017), the immunization page for Child #7 was not available for review. 2. The start date for Child #7 was listed as 08/28/2017. 3. Child #7 was observed in attendance on the date of inspection. Plan of Correction: The Parent of Child #7 shall be contacted and the immunization page shall be obtained and placed on file.
