Technical Assistance:
Discussed with provider to please sign proof of birth forms. Although shot records have been updated for Child #1, please speak with parent regarding more recent updates.

The following inspection took place between the hours of 9:45 am through 12:45. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 7 children (22 points) with 2 caregivers. A total of 2 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, napping, learning alphabets, infant feeding, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-730-D
Description: Based on observation, provider did not ensure that medications for household members were kept in a locked place. Evidence: Liquid ibuprofen was found on the bathroom counter, accessible to children.

Plan of Correction: Medication was locked up.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled with the child's full name and date. Evidence: 1. 1 of 6 bottle was not labeled with the child's full name and date. 2. 6 bottles were not dated.

Plan of Correction: I will label them.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of physical examination prior to child's attendance or within 30 days of attendance. Evidence: Child #1 (date of attendance 5/8/2017) does not have documentation of a physical exam.

Plan of Correction: I will ask the parent to have form completed.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider has not maintained a complete record for each caregiver. Evidence: The provider has not obtained documentation of two written reference for Caregiver #1.

Plan of Correction: I will ask her to get a reference.

Standard #: 22VAC40-111-50-A
Description: Based on record review and provider interview, the provider has not maintained a written record of children's attendance each day. Evidence: There was no record of attendance for 9/5/2017 and 9/6/2017. There were 7 children in attendance upon inspector's arrival.

Plan of Correction: I will start recording attendance.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, the provider has not obtained parent's signed acknowledgment of the receipt of the information required. Evidence: The record for Child #2 was missing a parent signature for the required written "Information for Parents."

Plan of Correction: Parent will fill it out.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the parent did not obtain [before the child's first day of attendance] signed acknowledgement of liability insurance. Evidence: The record for Child #2 was missing signed acknowledgement for liability insurance. (First day of attendance - 9/5/2017)

Plan of Correction: Parent will sign it.

Standard #: 22VAC40-111-280
Description: Based on observation, cleaning agents were not stored inaccessible to children. Evidence: The lower cabinet in the bathroom was kept unlocked with cleaning materials inside and accessible to children. Cleaning materials were also found above the toilet on the towel bar, and potentially accessible to children.

Plan of Correction: Locked cabinet.

Standard #: 22VAC40-111-510-C
Description: Based on observation, cribs were not spaced at least 12 inches from each other. Evidence: Upon arrival, the inspectors observed 3 cribs each with an infant sleeping inside. There was approximately 3 inches of space between each crib.

Plan of Correction: I will rearrange them differently.

Standard #: 22VAC40-111-530-B
Description: Based on observation, the provider did not maintain a tight fitting bottom cover for one crib. Evidence: Upon arrival, inspector's observed a 3 month old infant asleep in the crib. She was placed on a thin receiving blanket on top of a fitted sheet.

Plan of Correction: I will remove the receiving blanket.

Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was utilized by an infant while sleeping in a crib. Evidence: Upon arrival, inspectors observed an infant [10 months old] asleep in the crib with a stuffed animal.

Plan of Correction: I will remove the stuffed animal.
