Technical Assistance:
The licensing representative discussed the following with the director: Expired MAT training, Trash can being closer to changing table More cushion near swing and slide located in the playground area.

An unannounced renewal inspection was conducted today from approximately 10:15am-12:10pm. There were 9 children (20 points, two of which were the providers children) directly supervised by 2 caregivers. A sample of 7 children?s and 2 staff records were reviewed. The children were observed playing with puzzles, playing with provider, diapering and toileting, playing with blocks, coloring and playing dress up. There area for children was well maintained and the infant area contained age appropriate toys. No new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, I can be reached via e-mail at or by phone at 571-835-0599.

Description: Based on record review children's records did not contain required information. Evidence: 2 of 7 children's records were missing some required information. 1) Child A - Missing the emergency contact address of 2nd emergency contact person. 2) Child B - Missing the home address of the mother.

Description: Based on record review 16 hours of annual training was not completed. Evidence: Caregiver 1 could not provide documentation of the completion of 16 hours of annual mandatory training.
