Thank you for your time and assistance during the unannounced focused monitoring inspection conducted on September 14, 2017 between the hours of approximately 10:32 a.m. and 1:00 p.m. Twelve (12) children were enrolled and nine children were in the care of two caregivers on this date. One visitor/volunteer was also present in the home. Three children's records and three caregivers' records were reviewed and/or updated. No medications were administered on this date. Facility documents and posted information were observed/reviewed. The lunch provided on this date included chicken, macaroni and cheese, carrots, grapes, applesauce, crackers and milk. Program observation was completed. Observed activities included: indoor play (toys); stories; group game; restroom, diapering and handwashing procedures; lunch; and naptime. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. One previously cited violation (inspection date - 7/06/2017) was reviewed for compliance; no repeat violations were cited. The exceptions to compliance were noted on the accompanying violation notice. Standard #: 22VAC40-111-100-A Description: Based on review of two children's records and discussion with caregiver #1 on September 14, 2017, the facility failed to obtain a physical exam report for one child. Evidence: 1. Review of child B's record, on this date, determined that documentation of a physical exam report was not on file. The first date of attendance was 7/25/17. 2. Caregiver #1 confirmed the lack of a physical exam report in the child's record. Plan of Correction: A physical exam report will be obtained.
Standard #: 22VAC40-111-120-A Description: Based on review of caregiver #2's record and discussion with caregiver #1 on September 14, 2017, verification of age and references were not included in one caregiver's record. Evidence: 1. Review of caregiver #2's record, on this date, determined that verification of age and references were not included in the record. 2. Caregiver #1 confirmed that verification of age and references were not on file. Plan of Correction: Verification of age and references will be obtained and included in the caregiver's record.
Standard #: 22VAC40-111-90-C Description: Based on update of one child's record and discussion with caregiver #1 on September 14, 2017, the provider failed to obtain documentation of additional immunizations once every six months for one child under the age of two years. Evidence: 1. Update of child C's record, on this date, determined that an updated immunization record had not been obtained within the past six months (date of most recent immunizations - 9/07/2016). 2. Caregiver #1 confirmed the date of the last immunizations. Plan of Correction: An updated immunization record will be obtained.
