Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding maintaining records and notifications in the event of an emergency.

Comments:
A Subsidy Health and Safety inspection was conducted today, September 11, 2017 to review supplemental health and safety requirements. The inspection was initiated at 2:30pm and concluded at approximately 4:30pm. Four children were in attendance being supervised by the provider. Children were observed playing with toys and interacting with the provider. Administrative records and all areas in the home used by children were inspected. The provider's records and three children's records were inspected along with all subsidy requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on the inspection of the family day home's emergency preparedness plan, some of the required components were not listed.

Evidence:
During the inspection of the family day home's emergency preparedness plan, it was discovered the following procedural components were not listed under the evacuation and shelter-in-place sections of the plan.

1. Accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation; and
2. Method to ensure essential documents, including emergency contact information, attendance records, medications, and supplies are taken to the assembly point and relocation site.
