Technical Assistance:
Discussed requirements for new staff at the family day home and copies of the VDSS forms were provided. A meeting with zoning will occur on September 20, 2017 to hopefully increase capacity to 12 children. Several improvements were made to the deck, chimney, and stairs to meet zoning requirements.

Comments:
An unannounced monitoring inspection was conducted today from 12:00pm-1:30pm. At the time of inspection, 6 children (12 points) were directly supervised by the provider and two assistants. The sample size consisted of 6 children records and 2 caregiver records. There were no medications on site today and the provider is not MAT certified. Children were observed getting ready for nap, napping, and a few children were waking and playing quietly. Children bring lunches from home. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-A
Description: Based on observation, three diaper ointments were not labeled with the child's name.

Plan of Correction: I will label the ointments.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination within 30 days after the first date of attendance. Evidence: Child #1 (start date July 4, 2017) did not have a completed physical examination on file.

Plan of Correction: I will ask the parents for the physical exam.

Standard #: 22VAC40-111-120-A
Description: Based on review of caregiver records, Caregiver #1 did not have a record of orientation, emergency contact, name/address, references, and date of employment.

Plan of Correction: I will have her complete these today.
