Comments:
A Subsidy Health and Safety inspection was conducted on 09/18/2017 to review supplemental health and safety requirements. Observed 6 children (16 points) directly supervised by 1 caregiver. A sample of 4 children's records, 3 caregiver records, 2 household member records, and 2 pet record were reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, hand washing, diapering, and infant care. The exit interview was conducted with the provider. Inspection conducted from approximately 9:00am to 11:30am. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based upon documentation reviewed and interview, the vendor did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The attendance sheet for the week of September 11 did not document the arrival and departure of each child in care as it occurred.

Plan of Correction: I will start to do it.

Standard #: VENDHOM-000-029-A
Description: Based upon documentation reviewed, the family day home written emergency preparedness plan was incomplete. Evidence: The emergency preparedness plan provided for review did not contain lockdown procedures.
