SHSI monitoring conducted today with the provider and two assistants. Ten children were in care total of 24 points. Children were observed during free play. Four children's records and the provider and two assistants record's were reviewed. Inspection time 12:00am - 12:45am.
Standard #: VENDHOM-000-012-A Description: Based on observation provider and staff did not have documentation of completing the Virginia Pre- Service Training. Evidence: There is no documentation that the Provider or Assistant A and B completed the Virginia Pre- service training. Plan of Correction: We will take the training.
Standard #: VENDHOM-000-012-E-1 Description: Based on review of provider's and the assistant's record not all CPR trainings were not current. Evidence: Assistant A's CPR card is dated 7/21/15 . The card states it is valid for two years. Plan of Correction: We are signing up for a CPR training in October.
