An unannounced renewal inspection was conducted on September 26 2017 between 10:30am and 1:00pm. There were 9 children in the direct care of 2 staff members. The children were observed playing outside, being diapered, napping, washing hands and eating lunch. There is ample books and toys for the children to play with both inside and outside. Standard #: 22VAC40-111-800-B Description: Based on observation, the emergency plan was not documented as reviewed at least annually. Evidence: The date documented on the emergency plan of when it was last reveiwed is May 10th 2016. Plan of Correction: Will review with staff next week.
Standard #: 22VAC40-111-60-B Description: Based on record review, 2 of 4 children's records reviewed did not have all of the required information documented. Evidence: Child #1 is missing documentation of a yearly parent update. The last documented annual update is listed as 5/11/2016. Child #2 is missing documentation of an annual parent review. The last documented review was on 8/23/2016. Plan of Correction: I will talk with the parents today.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review, 2 of 2 staff records reviewed did not have documentation to prove that any providers in the family day home have current certification in cardiopulmonary resuscitation. Evidence: Staff #1 and Staff #2 both have expired cardiopulmonary resuscitation certifications that expired on 09/19/2017 Plan of Correction: I will complete the courses next week.
Standard #: 22VAC40-111-140-B-4 Description: Based on record review, 2 of 2 staff records reviewed did not have documentation to prove that any providers in the family day home have current certification in First Aid. Evidence: Staff #1 and Staff #2 both have expired cardiopulmonary resuscitation certifications that expired on 09/19/2017 Plan of Correction: I will complete the courses next week.
Standard #: 22VAC40-111-210-A Description: Based on record review, 2 of 2 staff members did not have the required annual training. Evidence: Staff #1 and Staff #2 do not have documentation of any training hours during the licensing year. Plan of Correction: Both staff have upcoming courses.
Standard #: 22VAC40-111-240-A Description: Based on observation, the areas and furnishing of the family day home are not maintained in a clean, safe, and operable condition. Evidence: Areas in the day care space have areas of chipping paint. Plan of Correction: Will paint by October 17th 2017
Standard #: 22VAC40-111-320-B Description: Based on observation, the provider does not have child-resistant protective covers on all electrical outlets that are accessible to children under 5 years of age. Evidence: In the main daycare space, 1 outlet is uncapped and in the crib room 4 outlets are uncapped. Plan of Correction: Was corrected at the time of inspection.
Standard #: 22VAC40-111-530-B Description: Based on observation, the cribs used in the day care do not have tight-fitting bottom covers. Evidence: In 2 of the 4 cribs, the bottom covers are able to be bunched up. Plan of Correction: I will order some online.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface is not non-absorbent. Evidence: The previously cited tear was fixed, but more tears are observed. Plan of Correction: I will replace it.
